Hailing from Ardrossan in North Ayrshire, James Cairns played for nearby Stevenston Thistle in the mid-1890s. His introduction to English football might have come with Burton Swifts during 1894-95 when he played seven times in the Second Division as an outside-left. It is certain, however, that he served Glossop North End in 1896-97 while they competed in the Midland League.
In the 1897 close season, James moved to Lincoln City but, in two months under the guidance of future Newton Heath manager James West, he was restricted to first-team friendlies or the reserves. At times he was pulled back from his position on the left wing to help out at left-half. In October 1897, he was released and joined Berry's Association, a Greater Manchester side from the Lancashire Combination.
This brought him to Newton Heath's attentions, and he was part of a massive recruitment drive in the summer of 1898. Standing 5'8" tall and weighing 11st 12lb, Cairns was included in the 'Colours' side to face the 'Whites' in a couple of pre-season trial games, and he was "conspicuous at outside-left".
Unfortunately, his stay with Heath was also a short affair. He made his debut in a friendly versus Chorley on the first day of September but could only get into the League team a single time: an encounter with Burslem Port Vale on October 8th. Cairns was selected in an unfamiliar inside-right spot yet was able to reflect on a 2-1 victory.
The rest of those first two months of 1898-99, the Scot was confined to reserve level football, in which he frequently excelled, scoring four goals. However, by November, he was back with Berry's, before returning to Stevenston Thistle in the 1899 summer.
